St John’s Hospice specialist palliative care accepts referrals for adults aged 18 years and over, who have complex needs associated with advanced, progressive, incurable, malignant or non-malignant disease and are living in the Doncaster locality.
Referrals for the inpatient unit are accepted from GPs, hospital consultants and their teams (often in liaison with the hospital palliative care teams), the community Macmillan nurse specialists and district and community nurses.
All referrals need to be made via the New referral form and fully completed with attached additional information as appropriate.
